Although it is known popularly as the “Carte Jaune” or “Yellow Card”, the color may vary from beige to light yellow. All booklets are authentic, not reproduced. Printed by the US Government and authored by the CDC It is not designed for COVID-19 vaccination. A special Covid-19 vaccine card, published by the CDC, will be issued to you by your vaccine provider after you have received the vaccine . The Certificate is used for recording routine and travel vaccinations and medications to prevent disease (prophylaxis). It provides official proof of Yellow Fever Vaccination for International Travel. Additional pages have space for your health history, regular medications, and allergies. The Yellow Card is recognized internationally and may be required for entry to certain countries where there are increased health risks for travelers. Before departing for a foreign destination, a traveler must visit a healthcare provider and receive the required vaccinations, which the doctor must list on the form per the instructions and sign and date. The Yellow Card must then be kept with the traveler's passport as he or she travels, since it serves as a medical passport of sorts as proof of vaccination, including booster shots.
